Dynamic Care provides Specialist Support Coordination to those who require a higher level of support coordination. Our highly skilled Specialist Support Coordination team will manage challenges in your support environment, and connect you with the services and providers that support your individual goals.

The NDIS define Specialist Support Coordination as time-limited support coordination required to support significant risks in a participant’s situation, to address the challenges in their environment, build capacity and resilience, and/or assist them by connecting to appropriate supports.

The following are examples of the higher level of support that our Specialist Support Coordination program can provide:

• psychological and mental health support
• accommodation arrangements
• ongoing support from a Social Worker
• accessing health, education or justice services.
